Abstract

Resistant Hypertension (RH) is a complex condition, characterized by persistently elevated blood pressure despite optimized therapy with three or more medications. This article describes the case of a 58-year-old man with a complex history of multimorbidity (hypertension, type 2 diabetes, obesity, and dyslipidemia). Despite follow-up in various specialties (Cardiology, Nephrology, General and Family Medicine), blood pressure control remained insufficient, with episodes of therapeutic non-adherence, medication intolerances, and suspected white-coat hypertension. The multidisciplinary approach proved essential, but insufficient without patient commitment. The case highlights the central role of Family Medicine (FM) in coordinating a condition of this complexity, ensuring efficient communication between levels of care, rigorous monitoring, and therapeutic education for the effective control of RH within a FM context.
